Supported Community Connections

What is the Supported Community Connections Program?

At PPCH, we recognize that individuals with intellectual and/or developmental disabilities want regular and meaningful access to their community. Our Supported Community Connections (SCC) program allows individuals to explore their community each day by participating in recreational activities, volunteer opportunities, and more.

Door-to-door transportation is provided by PPCH staff.

The SCC day program may include:

Volunteering with local non-profit organizations
Touring museums and nature centers
Making small purchases to promote money management skills
Touring local businesses to learn about potential work opportunities
Participating in recreation to promote an active, healthy lifestyle
and much more!
