What is the Colorado’s Community First Choice
(CFC) Program?
Community First Choice (CFC), also known as 1915(k), is an optional Medicaid program that allows Colorado to offer select home and community based attendant services and supports to eligible members on the State Plan, expanding these long-term care services to more Health First Colorado (Colorado’s Medicaid Program) members.
PPCH is proud to offer services under the Colorado First Choice (CFC) Waiver.
CFC provides select home and community-based attendant services and supports to a broader range of Health First Colorado (Colorado’s Medicaid program) members. Members have the option to self-direct their care or work with an agency to manage services, offering greater control and choice.
Participants must be 18 or older, Medicaid-eligible, and able to direct their own care or designate someone to help direct it.

- Medication reminders
